This is a 25 year-old female with a history of a silicone implant which was placed in Asia years ago who developed nasal asymmetry secondary to complications with the nasal implant. The patient noted a lack of projection from the implant and movement of the implant which created a 'crooked nose'. On exam, the implant was clearly displaced creating a loss of projection on profile and dorsal irregularities. Given the condition and the patient's desire to have more projection on profile, we recommended removal of the silicone implant and placement of a shape costal rib cartilage. The patient had an open rhinoplasty at our JCAHO accredited facility in Downers Grove, IL under general anesthesia. The rib cartilage was harvested from a 3 cm incision under the left breast. The rib was shaped to allow for more dorsal augmentation and give more tip projection. The silicone implant was removed via an open rhinoplasty approach and the rib graft was placed into the pocket. The nasal tip was shaped around the rib graft. The patient had an uneventful postoperative course. The rib graft was able to restore symmetry to the nose and give the patient more projection on profile and of her tip.
