The patient presented to the office with complaints of facial aging with jowls and loss of facial volume. She also had loss of definition of her neck with a 'turkey neck'. We recommended a short scar facelift with a corset platysmaplasty of the neck and fat grafting to the cheeks and lower eyelids with a pinch lower eyelid blepharoplasty for skin excision. She had the entire procedure under local (tumescent) anesthesia. We performed liposuction of the neck and jawline with a corset platysmaplasty. A short scar facelift was performed with retrotragal incisions and a SMAS plication with Quill sutures. Platelet rich plasma was injected into the face and neck before closure of the incisions. Microfat and nanofat was harvested from around the belly button and used to fat graft in the cheeks and the lower eyelids. Nanofat was added to the areas of the dark circles under her eyes to correct it and full out the volume loss. This combined with microfat grafting to the cheeks and a pinch blepharoplasty of the lower eyelids removing a small amount of skin allowed for a minimally invasive approach to rejuvenating the lower eyelids.
