This is a 59 year-old female with a 30 year history of smoking who presented to the office with complaints of facial aging. She had recently quit smoking 6 months prior. We recommended a short scar face and neck lift, autologous microfat and nanofat grafting to the face, laser resurfacing of the lower eyelids and perioral area, and platelet rich plasma (PRP) for the facelift and areas of laser resurfacing. The patient had the procedure under local (tumescent) anesthesia at our JCAHO accredited facility in Downers Grove, IL. The face and neck lift was performed using a retrotragal, short scar incision which hides well with minimal visibility. The neck was treated with a corset platysmaplasty. Blood was taken from the patient to create platelet rich plasma (PRP) using the Arteriocyte Magellan TruPRP System. The PRP was used in the facelift and necklift incisions to decrease postoperative bruising and swelling. Autologous fat was harvested from the periumbilical area and processed into nanofat for use in the tear trough area, and microfat for use in the cheeks and nasolabial folds. The Lutronic CO2 laser was used for laser resurfacing of the entire face including the lower eyelids and perioral area. PRP was used as a topical treatment in the areas of laser resurfacing to shorten the patient's recovery time. The patient had minimal bruising and swelling postoperatively and has a nice improvement in facial contour and overall facial skin texture and elasticity.
