This is a 57 year-old male who presented with a biopsy positive basal cell carcinoma of the right nasal sidewall. The basal cell carcinoma was excised and frozen section margin analysis confirmed the margins were negative for cancer. The patient had a 1 cm defect of the right nasal sidewall. The defect was reconstructed with a medially based bilobed transposition flap. The procedure was done under local anesthesia with the patient awake. The flap was rotated into the defect. At 6 months, the patient has maintained his nasal shape and has minimal visibility of the flap or the incisions.
