This 77 year-old patient had a basal cell carcinoma of the right medial cheek that was excised by a mohs surgeon leaving her with a 2.5 cm defect of her right medial cheek. We reconstructed her defect using a local rotation advancement flap which was inferiorly based and designed in her right nasolabial fold. This allowed for superior advancement of the flap to cover the defect while camouflaging the scar in the right nasolabial fold. The entire procedure was done under local anesthesia and the patient is cancer free at 6 months followup.
