This is a 30 year-old female who presented to the office with complaints of loss of breast volume after breastfeeding two children. On exam, the patient had bilateral deflated breasts with a lower pole constriction of both breasts. The patient did Crisalix 3D imaging software in the office to explore the ideal size and shape implant for her goals. She decided on Mentor 450 ccs Smooth Round High Profile Xtra Silicone Breast implants. The patient had a pre-existing nipple position asymmetry with the left nipple sitting higher than the right one as seen on her preoperative photos. The patient did not want any periareolar incisions to correct the nipple position asymmetry. She opted for bilateral inframammary incisions that would not be visible on the chest. The patient had the surgery in our AAAASF accredited office-based surgical center in Downers Grove, IL, The implants were placed in the submuscular position via an inframammary incision with the use of a Keller funnel for placement of the implant. Exparel (liposomal Marcaine) was injected around both breasts for better postoperative pain control and to decrease the need for postoperative narcotics. The patient had an uneventful postoperative course and was off all narcotics by postoperative day number three.
