The patient is a 39 year-old female who had a biopsy positive basal cell carcinoma (skin cancer) of the her right nasal sidewall and tip. She was recommended to have excision of the skin cancer in the office under local anesthesia. The cancer was excised using mohs technique and sent for frozen section margin analysis to determine that the margins were negative. Once the margins were determined to be negative about one hour later, the patient had a 2.3 cm defect of the right side of her nose. We discussed her options for reconstruction of the defect and given the size a left laterally based bilobed flap was recommended for reconstruction. The reconstruction was done on the same day as the excision with the entire process taking about 2-3 hours. After the nose was reconstructed with the local flap, the patient was sent home. She followed up on day five for suture removal. At three months, she had a very good cosmetic result with minimal distortion of the nasal shape and minimal scar visibility. The patient was happy with the result. The entire excision and reconstruction was done in our office in Downers Grove, IL under local anesthesia.
