This is a 21 year-old male who presented to the emergency room after a motor vehicle accident who struck the steering wheel with his forehead. He sustained a significant injury to his nose and lower forehead. On CT scan, he was diagnosed with a fracture of the anterior table of the frontal sinus, fracture of the naso-orbito-ehtmoid area with involvement of the nasofrontal ducts, and nasoseptal fractures. Given the obliteration of the nasofrontal ducts, the patient was recommended for cranialization of the frontal sinus, bone grafting of the nasofrontal ducts, a pericranial flap to line the cranialized sinus, and closed reduction of the nasoseptal fractures. This was performed through a frontal and temporal hairline incision. Notice the patient has correction of the lower forehead depression, restoration of the projection of the nasal dorsum and minimal visibility of the hairline scars on the postoperative photos.
